گلومرولونفریت کرسنتیک (هلالی) در بیمار با اندوکاردیت عفونی قلب راست

Abstract:
سابقه و هدف: اندوکاردیت عفونی بیماری است که بر روی دریچه های قلب وژتاسیون ایجاد می کند. نوع خاصی از آن در زمینه مصرف مواد مخدر تزریقی بروز می نماید. اکثر این بیماران، مردان جوان هستند. گلومرولونفریت هلالی یک عارضه نادر اندوکاردیت عفونی است که در عرض چند هفته تا چند ماه به سمت نارسایی کلیه پیش می رود. گزارش مورد: بیمار آقای 40 ساله با ادم، تهوع، استفراغ، تنگی نفس، تب و کاهش حجم ادرار مراجعه کرد. به دلیل نارسایی شدید کلیه و هیپرکالمی، دیالیز انجام شد. با توجه به تب و سوفل قلبی از بیمار اکوکاردیوگرافی بعمل آمد که وژتاسیون بزرگی روی دریچه تریکوسپید معلوم گشت. جهت تعیین نوع ضایعه کلیوی، بیوپسی انجام شد. پس از مدتی مشخص شد که بیمار معتاد به مواد مخدر تزریقی می باشد. بیوپسی حاکی از گلومرولونفریت کرسنتیک (هلالی) فیبروتیک بود. به دلیل پیشرفته بودن بیماری، بیمار در برنامه دیالیز دائم قرار گرفت. نتیجه گیری: با توجه به اینکه در 22% از اندوکاردیت های عفونی، ضایعات گلومرولی بوجود می آید و در این بین نوع گلومرولونفریت هلالی نادرترین بوده و به نوعی بدترین پیش آگهی کلیوی را دارد، شک بالینی به مشکل کلیوی در حین اندوکاردیت عفونی می تواند در جهت مراقبت های لازم برای حفظ هرچه بیشتر این عضو کمک کننده باشد.
